Look at your shore power cord. If it has a large round plug with three prongs (one thick, curved, straight), that's a 30-amp TT-30 — apply for the 30A. If your plug has four rectangular blades in a square pattern, that's a 50-amp NEMA 14-50 — apply for the 50A. Not sure? Select "Both" on the application.
